Manager, Patient Navigation Job Title: Manager, Patient Navigation Reports To: Executive Director, Research Engagement Location: Remote Start Date: 2022 NephCure Kidney International’s mission is to accelerate research for effective treatments for rare, protein-spilling kidney diseases, and to provide education and support that will improve the lives of those affected by these conditions. Position Summary The Manager of Patient Navigation will provide health navigation services to NephCure constituents, including patients and care partners. This person will build working relationships with rare kidney disease patients in order to solve problems, locate resources, and support positive health behaviors and outcomes. The Manager of Patient Navigation is committed to assisting the patient by identifying challenges and barriers and providing critical resources to help them navigate through healthcare services and systems. As a member of the NephCure team, they will work in a fast-paced environment that values collaboration, innovation, diverse perspectives, and, most of all, a relentless determination to help those with rare kidney disease protect their kidneys and live their best lives. Primary Responsibilities Oversee development and execution of NephCure’s Patient Navigation Program. Provide care navigation, treatment support, and peer and clinical trial support to help individuals move through both payer and clinical spaces. Investigate, assess, and plan interventions to help patients cope with medical, social, emotional, economic, and environmental barriers in their health care. Determine, prioritize, provide, and/or arrange for needed internal and external services/interventions. Identify resources and services for patients to overcome challenges and barriers to care. Demonstrate the ability to develop and maintain professional relationships with resource providers (organizations that provide patient services), NephCure Specialists (rare kidney disease specialists), and their teams. Demonstrate ability and confidence in communication skills used in motivational interviewing. Develop resources and how-to guides for frequently requested topics such as insurance appeals, financial aid, getting a second opinion, etc. Ensure that all patients in need receive timely communication and follow up to include accurate record keeping. Represent NephCure in relevant hearings, meetings, partner coalitions, and conferences. Other Qualifications Licensed Clinical Social Worker or Licensed Master Social Worker. Minimum of two to four years of healthcare-related case management or social work experience. Top candidates will have experience working with a variety of private and public insurance sectors/companies. Experience in kidney health a plus. Understanding of the healthcare system, insurance processes, and how to influence health outcomes. Strong interpersonal skills and ability to form strong relationships. Ability to manage and track a large amount of information, stay organized, and use a systematic approach to information management. Strong writing skills and presentation skills with an understanding of writing for an organizational voice is highly preferred. Excellent communication skills and demonstrated ability to work as a leader and as part of a team, inspire others, and align team members and stakeholders toward agreed direction, goals, and objectives. Willingness to work a flexible schedule with minimum supervision. Ability to work occasional nights and weekends to support related activities. Proficient with MS Office computer software including Word, Excel, Outlook, and PowerPoint. Experience in grant writing a bonus. Ability to travel as needed. Valid driver’s license and reliable transportation. Work Environment & Considerations Candidates may live anywhere in the U.S. and work remotely.
